Residential Nurse (Licensed Practical Nurse, LPN)

The Residential Nurse coordinates and provides for the medical needs of persons with disabilities (referred to as “individuals” for the remainder of this document), in Bear Creek Services’ residential settings. This position is responsible for integrating health needs into the person-centered plan of individuals. 

Essential Duties and Responsibilities: 
Leadership 
  • Model the agency mission, vision, and values in all interactions. 
  • Promote person-centered outcomes. 
  • Maintain confidentiality of individual and agency issues. 
  • Participate in policy/procedure developments as requested. 
  • Teach medication administration classes, as necessary. Provide ongoing support to proper dissemination of medication, per regulation and policy. 
  • Completes all required training to maintain nursing and 245D licensing requirements.
  • Work collaboratively with others to solve problems. 
  • Approach others in a professional, tactful manner and treat others with respect and consideration regardless of their status or position. 
  • Understand, follow, and model all licensing regulations as related to nursing and disability support services.  
Program Oversight 
  • Work in consult with the Program Director. 
  • Attend all annual, semi-annual, and person-centered meetings as scheduled. Prepare required paperwork per statute. 
  • Accompany individuals on medical, dental, and psychiatric appointments; come prepared with all necessary documentation. Following up on all lab or test results with providers and verify any new or changed medications and/or treatment orders. 
  • Provide transportation as needed. 
  • Schedule appointments as required by the individual health needs assessment and person-centered plan.
  • Meet with dietician where necessary. Ensure specific diet plan and appropriate menu substitutions for everyone are adhered to, as required by medical need and/or regulation. 
  • Meet with pharmacist consultant and review personal information per regulation.
  • Provide oversight for medication handling, per regulation and person-centered plan. 
  • Ensure weights and vital signs are completed, per person-centered plan.
  • Complete body checks per person-centered plan.
  • Follow regulation as outlined in State and Federal statutes and rules. 
  • Provide the necessary supervision in cooperation with Program Managers, according to the Coordinated Service and Support Plan (SUPPORT PLAN) and person-centered plan. 
  • Participate in the on-call rotation and perform on-call responsibilities. 
  • Perform direct care responsibilities as needed. 
  • Work cooperatively with Program Managers to ensure person-centered plans and medical needs are understood and followed. 
Communication and Reporting 
  • Ensure that all medical consents are obtained and kept current. 
  • Ensure medical documentation, records, and reports are complete and timely. 
  • Communicate medical changes to all members of an individual’s team, including guardian(s), family members, county case manager, and all appropriate Bear Creek Services support staff. 
  • Maintain timely and effective communication across all media (electronic, phone, verbal). 
  • Ensure that incident reports (General Event Reports) are completed in a timely manner and per regulation. 
  • Participate in program and health service meetings as scheduled. 
  • Provide direction and feedback to staff on an on-going basis. 
  • Ensure individual medical records are complete, in chronological order, and in good condition.

Experience and Educational Requirements:
Registered Nurse/Licensed Practical Nurse (RN/LPN) graduate from an accredited school.
Experience with people with developmental disabilities desirable. 

Licensing and Certifications:
Current Licensure with the Minnesota State Board of Nursing.
Valid Minnesota driver’s license and proof of current automobile insurance.
Current CPR certification.
